I've got a set of MR954II's I think that the number for these CD horns. Mine came with 299-8A drivers, but I never seen one with a small format (1") driver on them.

Is this a homemade adaptor or is this something that's available or was?

well, Altec sold all those parts.

I think the driver is older than the horn, and in between is a 21216? if I have the number correct...

Even if the driver is older than the horn, it was not uncommon for Altec dealers to have dated stock...so it COULD have been purchased that way.

Was it a combo Altec offered? No. It would have been put together from parts, either by a dealer or end user.

The MRII594A was shipped with the flat, square plate. The plate is a place for mounting brackets to attach. These horns are extremely tail-heavy with drivers mounted and a solid mount was required when suspending them. The 21216 is a 1"-to-1.4" throat adapter that fits the small format driver.
