I think David may have some pretty f**ked up ideas of how affection is bestowed or withheld due to his and Darcy'd illicit relationship.
I bet this comes into play.
And I bet this is part of the reason wy Katchoo is treating him the way she is.

Both Baker sisters called Katchoo "Cinderella." Tambi called her that in the Galleria in 1996, and Bambi called her that in the Florence Hotel penthouse a day later. Tambi also used the name in the hospital in 1998.
