Go to the top of your Paypal page, and click on resolutions. They will walk you through the dispute process.
I filed a case, and won. I was sold a Samantha doll, and throughout the entire auction it was described as Samantha. It turned out to be an AGOT. The seller refused to do anything about it, like refund my money, or even a partial refund.
I went to Paypal, and they found in my favor. She agreed to a refund of 1/2 price, and I got to keep the doll.
P.S. She left me negative feedback, which I expected.

No, it's not hard. I won my case, but I did not want to return the doll -- so she kept the money, and I kept the doll. She was a serial scammer, so I mainly just filed the claim to help get her kicked off eBay and to buoy the other PayPal claims against her.
One tip: type out your explanation in Word and then cut and paste it into the section where you are supposed to explain exactly why the item is not as described. If you take too long to type in the text box, PayPal will time you out, and then you have to log in again and start all over. My seller worded her auction pretty carefully to indemnify herself, so I had word my claim very carefully as well (which took some thought).
